An NP note is a clinical handoff, a legal record, and a record of professional judgment—not a clerical afterthought. In Canada, the duty to document comes from the applicable provincial or territorial regulator, health-information legislation, employer policy, and the standard of care. The record should allow another clinician to reconstruct what was known, what was done, how the patient responded, and what follow-up was arranged. The safest record is timely, attributable, objective, and secure. Document when care is provided or as soon as possible afterward; never chart care in advance. The CNO’s 2025 Documentation practice standard organizes this work around Communication, Documentation requirements, and Information security. The same core habits apply across Canadian settings, while local rules determine details such as billing, retention, controlled-substance monitoring, and access to records. Those habits become more demanding—not less—when the NP is making diagnostic, prescribing, and coordination decisions.
