Pediatric vital signs differ profoundly from adult normals and change continuously with age. A nurse who applies adult vital sign standards to children will misinterpret normal findings as abnormal and, more dangerously, fail to recognize early signs of deterioration. For the REx-PN exam, vital sign questions test: (1) whether a given vital sign is normal for the child's age; (2) the correct temperature measurement method for the age; and (3) the blood pressure formula for estimating minimum normal systolic BP. These are frequently tested in scenarios where the candidate must decide whether a vital sign requires escalation. On the exam, writers often pair stable-sounding options with unstable dataโnotice the mismatch before you commit. If the stem names a license or role, reread that line; scope errors are classic trap answers even when the clinical topic is familiar.
