Thrombolytics are clot-dissolving medications used when a fibrin-rich clot is causing immediate organ threat. The same action that can restore perfusion can also cause catastrophic bleeding, so the clinical question is never simply whether alteplase or tenecteplase dissolves clot. The question is whether this patient, at this time, has an indication strong enough to justify bleeding risk. For CNPE/NP readiness, the learner must connect the emergency presentation with timing, imaging, contraindication screening, baseline neurologic or hemodynamic status, and diagnose the emergency pattern, select or recommend reperfusion pathways within prescriptive authority, screen contraindications, coordinate transfer, and plan follow-up.
