Suicide Precautions Observation

Suicide precautions observation is an active safety intervention, not simply placing a patient near the nurses’ station. The level of observation should match the patient’s current risk and the hazards of the physical environment. Its purpose is to detect a dangerous change early, prevent access to lethal means, and make immediate assistance possible if the patient acts. Suicide risk is dynamic. A patient may deny suicidal thoughts at admission and later develop intent after receiving upsetting news, becoming intoxicated, learning of a discharge delay, or moving to a less structured setting. A calm or cooperative presentation can reflect genuine improvement, but it can also reflect concealment or a decision to act. The nurse interprets the patient’s words alongside behavior, access to means, recent self-harm, and the surrounding circumstances. For the LVN/LPN, practice remains within state law, employer policy, and the assigned plan of care.
