Medication Administration Safety

Safe medication administration requires verification of the Five Rights before every dose. The classic rights are the right client, right drug, right dose, right route, and right time. They are safety goals, not a complete procedure: a nurse can match the MAR and still administer an inappropriate medication if the order is unclear, the client’s condition has changed, or a system safeguard is bypassed. The LPN/LVN first verifies a current client-specific order or other valid authority and confirms that the medication and route are permitted by the state nurse practice act, facility policy, required supervision, and the nurse’s competency. Hold and clarify an order that is incomplete, ambiguous, inconsistent with the client’s assessment, or outside the nurse’s authority; never guess. Safety is layered through two identifiers, repeated label checks, standardized storage and preparation, barcode medication administration when available, selective independent double checks, and assessment of the client’s response after the dose.
