Growth Charts and Parental Teaching

A growth chart is a record of trajectory, not a grade. One plotted value describes where a child measures today; serial, accurately obtained values show whether weight, length or height, and head growth are following an expected pattern. A stable child at a low or high percentile may be growing normally, whereas a sustained downward shift after previously typical growth needs assessment. The nurse’s sequence is straightforward: verify the measurement, select the correct chart and index, compare with prior data, assess the child’s clinical context, and report a meaningful deviation. The chart supports clinical judgment; it does not replace feeding assessment, developmental assessment, or provider evaluation.
