NCLEX-PN
A missed priority in Confidentiality can delay recognition of deterioration or allow preventable harm to continue. Safety focus: CHAMPS for confidentiality exceptions: Child/elder abuse reporting, HIV/communicable disease, Abuse of dependent adults, Motor vehicle impairment (state-specific), Protection duty (Tarasoff), Suicidality/self-harm imminent. Tarasoff WARN: Warn victim, Alert law enforcement, Refer for hospitalization, Notify (document everything).
In a Confidentiality item, explain the first cue you noticed, the complication it predicts, the nursing action within scope, and the finding that proves the response worked.
Tarasoff applies to IDENTIFIABLE third parties — a general statement of 'I want to hurt someone' without identification does not trigger the same specific duty This keeps Confidentiality reasoning tied to client safety instead of recall-only studying.
