A missed priority in Attribution Theory can delay recognition of deterioration or allow preventable harm to continue. Safety focus: FAE = Fundamental Attribution Error: blame the PERSON not the PLACE (for others). Actor-Observer: I blame the STAGE, you blame the STAR.
