Clinical meaning
The NP applies diagnostic criteria logic using Bayesian reasoning — a systematic framework that adjusts the probability of disease based on sequential clinical information. Effective diagnostic reasoning requires understanding how pre-test probability, test operating characteristics, and clinical context interact to guide decision-making. Pre-test probability is the estimated likelihood of disease BEFORE testing, determined by history, physical examination, risk factors, and prevalence in the patient's population. Pre-test probability dramatically affects the meaning of test results. The same positive test result has very different implications in a high-prevalence population versus a low-prevalence population. Test operating characteristics: Sensitivity (true positive rate) = proportion of patients WITH disease who test positive. Specificity (true negative rate) = proportion of patients WITHOUT disease who test negative. A highly sensitive test is best for RULING OUT disease (SnNOut: Sensitivity Negative rules Out). A highly specific test is best for RULING IN disease (SpPIn: Specificity Positive rules In).
