Zollinger-Ellison syndrome (ZES) results when a gastrinoma releases gastrin without normal control. Gastrin keeps parietal cells stimulated, so gastric acid remains excessive and ulcers may recur despite usual treatment. The bedside pattern is more distinctive than any single symptom: multiple or refractory ulcers, disease beyond the usual duodenal location, secretory diarrhoea, weight loss, or findings suggesting multiple endocrine neoplasia type 1 (MEN1). Care therefore combines sustained acid suppression with careful biochemical interpretation, tumour assessment, and surveillance for acid-related emergencies.
