Gravidity, Parity and GTPAL Notation

GTPAL is a compact description of reproductive history, but it is not clerical shorthand. It alerts the team to histories that may change antenatal surveillance, delivery planning, or postpartum preparation: a prior preterm birth, stillbirth, cesarean birth, pregnancy loss, multiple gestation, or high parity. The notation cannot replace a narrative history. A client with a prior cesarean, severe postpartum hemorrhage, neonatal death, or preterm birth needs those events described, not merely counted. GTPAL identifies the pattern; the history explains the risk. At the first prenatal assessment, an experienced nurse notices whether the numbers fit the story. A gravida 4 client with only two completed outcomes documented may have an unrecorded loss, an unclear gestational age, or a previous multiple pregnancy that was counted inconsistently. Resolve that discrepancy before risk screening relies on the record.
