Oxybutynin is an antimuscarinic. It blocks M3 muscarinic receptors in the detrusor muscle, reducing the acetylcholine-driven contractions that make the bladder squeeze before it is ready. The bladder can then store urine longer, so urgency, frequent voiding, and urge urinary incontinence may decrease. The medication treats involuntary detrusor activity; it does not treat every reason a patient may void frequently or leak urine. A sudden compelling urge followed by leakage suggests urge incontinence and fits the drug’s therapeutic target. Leakage with coughing or lifting suggests stress incontinence. Frequent small voids with hesitancy, weak stream, suprapubic fullness, or continuous dribbling suggest incomplete emptying or overflow, which oxybutynin can worsen. Large urine volumes associated with marked thirst, hyperglycaemia, or diuretic use point toward polyuria rather than overactive bladder. That distinction is more than terminology. An experienced nurse notices the pattern of the voiding complaint before focusing on the medication, because suppressing detrusor contraction in a patient who cannot empty the bladder creates a safety problem.
