Endocrine disorders often announce themselves through a mismatch: the person’s intake, activity, temperature, body weight, energy level, or fluid balance no longer fit their usual pattern. The first task is to decide whether that mismatch is an emergency. Check airway, breathing, circulation, level of consciousness, vital signs, and bedside glucose when symptoms could reflect hypo- or hyperglycemia. Confusion, seizure, syncope, new focal neurologic findings, severe weakness, persistent vomiting, Kussmaul-type deep respirations, marked dehydration, or hypotension require urgent action while the assessment continues. Hypoglycemia may resemble anxiety, intoxication, stroke, or acute delirium. Adrenergic symptoms—sweating, tremor, palpitations, hunger, and anxiety—occur as the body releases catecholamines in response to falling glucose. Neuroglycopenic symptoms—difficulty concentrating, behaviour change, weakness, blurred vision, confusion, seizure, or reduced consciousness—mean the brain is receiving inadequate glucose and raise the priority immediately. Hyperglycemia usually develops more gradually. Polyuria, thirst, dry mouth, fatigue, blurred vision, recurrent infections, and unintentional weight loss point toward sustained glucose elevation. Glucose in the urine pulls water with it, producing osmotic diuresis; the person may be profoundly volume depleted even while drinking constantly. Vomiting, abdominal pain,...
