Scope of Practice and Unsafe Orders

An order is a clinical instruction, not a key that unlocks every nursing action. Before administering a prescription medication or carrying out a controlled act, an RPN must confirm two separate questions: Is there valid authorization for this client and this action? and Can I safely perform it within my scope, competence, setting, and current assessment of the client? A client-specific order from an authorized prescriber is required for prescription medication administration. Depending on the applicable provincial rules, authorized prescribers may include a physician, Nurse Practitioner, dentist, midwife, pharmacist, or RN with prescribing authority. The order does not replace nursing judgment. An incomplete order, a change in the client's condition, or a task outside the RPN's authority requires a pause, clarification, and escalation rather than an educated guess.
