Anticoagulants

Anticoagulants matter in REx-PN preparation because they require the learner to protect the patient from two opposite harms at once: thrombosis when clot prevention is inadequate and bleeding when anticoagulant effect or patient vulnerability becomes unsafe. The safest answer is rarely just give it or stop it. It depends on the indication, current bleeding cues, clot risk, renal and hepatic function, pregnancy context, procedure timing, interacting medicines, and whether the learner is acting within focused assessment, safe administration, patient teaching, documentation, and timely reporting.
